The story revolves around Emma Mancini, a 20-year-old university student aspiring to become a doctor, whose life takes an unexpected turn when her family's ties to the mafia catch up with her. Her father, Roberto Mancini, is a former Canadian mob boss who is now terminally ill with Stage 4 cancer. Unbeknownst to Emma, her brother-in-law, Fausto Ravazzani, and other members of the 'Ndrangheta, have been involved in a long-standing feud with Don Buscetta of the Sicilian Cosa Nostra.
One day, as Emma returns home from university, she encounters three menacing men who have bypassed her father's guards and entered the house. Led by Bernardo Virga, the capo di tutti capi (boss of all bosses) of the Sicilian Cosa Nostra, they inform Emma that she is to be married off to Giacomo Buscetta, the new Don of the Buscetta family, as part of a plan to settle the ongoing conflict between the two mafia families. Threats against her father's life compel Emma to comply, despite her reluctance and fear.
Emma is taken to Palermo, Italy, where she is forced into a hasty wedding with Giacomo Buscetta, a man known for his violent past, including professional fighting and murder. Giacomo, too, is coerced into this marriage by Virga, who holds a secret over Giacomo involving his sister, Viviana. Despite their mutual disdain for the situation, Giacomo and Emma find themselves bound together, not just by the ceremony but by the demands placed upon them by the mafia leadership.
In an attempt to satisfy Virga's requirement for proof of the consummation of the marriage, Giacomo fakes the evidence, avoiding physical intimacy with Emma. The novel explores the complex dynamics between the newlyweds, as well as the pressures and dangers of the mafia world they are both unwillingly part of. As they navigate their new reality, both Emma and Giacomo must confront the implications of their actions and the future that awaits them, all while trying to protect those they care about from the violent consequences of their decisions.
